To retain my services, please cut and paste this Contract form into your word processor (or ask me to send you a copy), complete the dollar amounts, sign and mail it to me with your check or money order along with your detailed request and any supporting documentation or recitation of known facts that you believe may assist me in my research:

CONTRACT
Enclosed is a check payable to "Michael S. Ramage, J.D." in the amount of $___________ ($U.S.), (5 hour minimum) as a retainer. I ("Client") engage the forensic genealogical services of Michael S. Ramage, J.D., CGsm (“Researcher”), 720 Argyle Road, Wynnewood, PA 19096, and I authorize him and his associates to conduct forensic genealogical research on my behalf, for an amount not to exceed $ __________ (time and expenses), as per my instructions (state in detail on a separate page). I have provided to Michael S. Ramage, J.D., CG, all pertinent information in my possession on the subject to be researched. Client acknowledges that Researcher can make no guarantees regarding what information, if any, may be found, although every reasonable effort will be made by Researcher to find the desired information within the time allotted by Client.
Michael S. Ramage, J.D., CG, agrees to provide a detailed report or affidavit, covering all relevant sources checked during the research, with analysis, and a detailed report of time consumed and expenses incurred. The report or affidavit will be mailed to the person whose name and address appears below within a reasonable amount of time of receipt of the retainer and fees due the Researcher. Client gives Researcher permission to submit such report and related files for renewal application purposes only to the Board For Certification of Genealogists.
The Researcher's work will be charged as follows: $100.00 per hour plus out-of-pocket costs including but not limited to photocopies, long distance telephone charges, postage and handling, travel, repository entrance fees, lodging (when reasonable and necessary), database fees and the like. Balance in full is due before final report or affidavit will be sent to Client.
Client authorizes Michael S. Ramage, J.D., to obtain vital records as may be necessary to establish and identify next-of-kin and collateral kindred of estate's deceased or real estate owners who are deceased or missing. A photocopy of this authorization shall have the same force and effect as the original.
